Parents/guardians can pick up Registration Packets at the District Administration Office or print the electronic version. Completed packets along with the required documents listed below can be emailed to the District Registrar or an appointment can be made to bring everything in. When returning the registration documents to the District Administration Office, please allow time to review at the office. All documents under the Registration Requirements below must be submitted together. Registration cannot be completed unless all requirements are provided. All applicable forms in the Registration Packet must be physically or electronically signed.  

Registration Requirements:  The following documentation is required.
   

If applicable, these will be due to the school office prior to or on the first day of school: 

Physical Examination (Click link for form)  Private Physician Form unless designated on Student Health History to be done by school physician at no charge for students entering grades K, 6th or 11th. 

Dental Examination (Click link for form)  Private Dentist Form unless designated on Student Health History to be done by school dentist at no charge for students entering grades K, 3rd or 7th.
